Body piercing will not be performed on anyone under the age of eighteen unless the following is provided and agreed to: The written and notarized consent of the legal guardian The consent must indicate the part of the person’s body that is to be pierced The individual’s legal guardian must be present at the body piercing studio at the time the piercing is performed An affidavit must be executed stating on whom the body piercing is to be performed Evidence must be presented of the person’s identity to whom the body piercing will be performed on Evidence must be presented verifying the status as to the legal guardian 2. We are prohibited from and will not perform body piercing to anyone whom we suspect is under the influence of drugs or alcohol I, _______________________________________, hereby certify by my signature below, that I am the legal guardian of the minor named below, competent in all respects, and have read and understand this release and that I am not under the influence of alcohol or drugs.
